Survey re Local Pharmaceutical Services from Warwickshire County Council

Do you use any of the local pharmacy services in Coventry and Warwickshire?

If so please help us to improve the pharmaceutical service by taking part in a survey and sharing your experiences of using them?

Coventry City Council and Warwickshire County Council are conducting the survey, which is officially known as the Pharmaceutical Needs Assessment (PNA), on behalf of the Health & Wellbeing Boards in Coventry and Warwickshire to assess the current and future needs of local pharmacy services available across the patch.

If you require a paper copy or need this information in another format or language, please email your name and address to PHAdmin@warwickshire.gov.uk(External link) or call 01926 731 484 and leave a message stating your name, number, the request for a Pharmacy survey and the address you’d like the survey sent to. We will then send you a paper version with a prepaid return envelope.

Your views are confidential and the findings from the survey will be used to inform the PNA.

The deadline for responses is January 31, 2025.
